Gallbladder–Heart Syndrome Presenting as Intermittent Left Bundle Branch Block: A Case Report

Die Zhang, Yi-Sheng Wang, Tian-Shu Yang

A 78‐year‐old woman was admitted with recurrent episodes of chest tightness and palpitations. During symptomatic episodes, the electrocardiograms (ECGs) demonstrated intermittent left bundle branch block (LBBB), which resolved spontaneously after symptom relief. Serial cardiac biomarkers remained within normal limits, and coronary angiography excluded significant coronary artery disease. Abdominal ultrasound and computed tomography revealed cholelithiasis. Following anti‐inflammatory treatment for biliary inflammation, the patient’s chest discomfort and palpitations markedly improved, and subsequent ECGs showed no recurrence of LBBB. This case raises the possibility that gallbladder pathology may contribute to transient electrophysiological disturbances through a proposed viscerocardiac reflex mechanism, potentially contributing to intermittent LBBB in the absence of obstructive coronary artery disease. Gallbladder–heart syndrome may be considered in selected patients with unexplained intermittent LBBB after acute coronary syndrome has been excluded, particularly when gastrointestinal symptoms are present.
