iCARE4CVD is bringing together multiple stakeholders to transform cardiovascular care, using secure data infrastructure, harmonised health information and AI-supported models to help identify what works best, for whom, and when. Cardiovascular diseases affect 85 million people in Europe, but no two patients are alike. The same disease has different causes, and patients with the same diagnosis can have different clinical courses, risks, symptoms and responses to treatment.
