From data literacy to AI-integrated Laboratory 4.0

Background
Laboratory medicine is undergoing a profound transformation towards a completely digitized and connected “Laboratory 4.0” driven by advances in laboratory automation, interoperability, and artificial intelligence. At the same time, it also faces challenges such as declining funding, a shortage of qualified laboratory professionals, increasing workforce constraints, and regulatory requirements. Digital competence thus becomes a crucial skill for addressing these challenges.

This opinion article summarizes a series of papers published primarily by authors from the German Society for Clinical Chemistry and Laboratory Medicine (DGKL) Section on Digital Competence and Artificial Intelligence (AI). Six subject areas are examined: (1) digital competence and data literacy; (2) open-source software tools; (3) interoperability standards; (4) total laboratory automation; (5) artificial intelligence applications across the pre-analytical, analytical, and post-analytical phases and (6) large language models with their capabilities and limitations. The regulatory implications of the EU AI Act and its intersections with the IVDR are discussed in the relevant sections.
Summary
Based on the sections work and other recent articles addressing these six topics mentioned above, we have identified four interrelated areas of action: (1) building digital and AI literacy among the laboratory workforce; (2) adopting open-source tools and interoperability standards as foundations of the fully connected laboratory; (3) deploying AI with appropriate safeguards, critical evaluation frameworks, and regulatory compliance; and (4) developing explicit institutional strategies that integrate cultural, medical, hardware, and software dimensions.
Outlook
It requires a systemic approach in which competence development, infrastructure investment, regulatory compliance, and clinical leadership are pursued in parallel. Laboratory professionals who engage actively with this transformation will be best positioned to ensure that digital innovation enhances rather than compromises the quality, safety, and consultative value of clinical laboratory medicine.
