Abstract

Background/Introduction

Ablation workflows continue to evolve as new technologies are approved and adopted. Here we summarize the first real-world cases of a novel dual-energy radiofrequency (RF)/pulsed field ablation (PFA) ablation catheter without protocol specifications.

Purpose

The objective of this analysis was to summarize first cases using the TactiFlex Duo Ablation Catheter in a real-world cohort without protocol specifications.

Methods

Acute, observational data from the first 17 procedures using TactiFlex Duo was collected from 5 European centres from January-February 2026.

Results

The most common indication for ablation was paroxysmal atrial fibrillation (PAF) (47.1%, 8/17), followed by atypical atrial flutter (AFL) (23.5%, 4/17), persistent atrial fibrillation (PersAF) (17.6%, 3/17), typical AFL (5.9%, 1/17), and atrioventricular reentrant tachycardia (AVRT) (5.9%, 1/17). FPI was achieved in 100% of cases (13/13). Most cases used deep sedation with propofol (88.2%, 15/17); the remainder used general anesthesia with a muscle paralytic (5.9%, 1/17), and conscious sedation without propofol (5.9%, 1/17). Of the 16 cases that reported the ablation modality used, 43.8% used both RF/PFA (7/16), 37.5% used PFA only (6/16), and 18.8% used RF only (3/16). The average number of PFA ablation lesions was 48.4±25.2 in PAF, 77.7±28.5 in PersAF, and 66.3±36.3 in atypical AFL. A summary of lesion set location by modality can be found in Figure 1. Most cases used the Advisor HD Grid X mapping catheter for map creation (58.8%, 10/17), followed by Advisor HD Grid (29.4%, 5/17), a circular mapping catheter (5.9%, 1/17), and the ablation catheter (5.9%, 1/17). The average procedure time was 86.4±31.0 min in PAF, 139.0±14.1 min in PersAF, 30.0 for typical AFL, 105.3±16.2 for atypical AFL, and 70.0 for AVRT. Cough response and movement response was reported in 38.5% (5/13) and 15.4% (2/13) of cases, respectively. There were 0 reported incidents of renal compromise.

Conclusion

These results highlight real-world use patterns of the TactiFlex Duo Ablation Catheter, supplementing acute results reported from clinical studies. Continued monitoring of use trends with dual-energy catheters is crucial, especially as the adoption of PFA in cardiac ablation expands.
