Background

To compare postoperative visual, topographic, and endothelial outcomes between femtosecond laser-assisted mushroom-designed penetrating keratoplasty (FAK) and suction trephination-assisted penetrating keratoplasty (PKP).

Patients and methods

A prospective, nonrandomized comparative study was conducted on 30 eyes divided into two groups: mushroom FAK and suction trephination-assisted PKP. Outcome measures included best corrected visual acuity, topographic astigmatism, keratometry (Km), CCT by Pentacam, and ECD, hexagonality, and CV by specular microscopy.

Results

At both 6 and 12 months postoperatively, the FAK group demonstrated significantly better best corrected visual acuity and lower topographic astigmatism compared with the conventional PKP group ( P <0.05). The CV was significantly lower in the FAK group at both follow-up visits, indicating more favorable endothelial morphology. No statistically significant differences were observed between groups regarding Km, CCT, ECD, or hexagonality.

Conclusion

FAK may provide faster visual recovery, lower postoperative astigmatism, and improved endothelial cell morphology during the first postoperative year compared with suction trephination-assisted PKP.
