A 13-year-old boy underwent FDG PET/CT to evaluate fever of unknown origin, which demonstrated extensive abnormal activity around joints of all 4 extremities and probable myopericarditis. Subsequent rheumatologic evaluation, including cardiac auscultation and echocardiography, identification of subcutaneous nodules, and elevated antistreptolysin O titers, established the diagnosis of acute rheumatic fever (ARF). Symmetrical polyarthritis with enthesitis-myotendinitis is an uncommon manifestation of ARF.
