The occurrence of severe aortic valve stenosis of calcified tricuspid aortic valve in four siblings at the age of nearly 80 years may give a clue to a genetic cause of the disease. All patients had prior to diagnosis several years of symptomatic angina, syncope and impairment of left ventricular function and even coronary artery disease. None of the patients had a history of rheumatic fever or endocarditis. Only in 2 patients was it possible to perform aortic valve replacement in time..
