Abstract

Background

The Kyoto Heart Failure Network Council (khfnet) operates within medical institutions in Kyoto Prefecture, sharing common evaluation and guidance objectives with the aim of improving heart failure prognosis.

Objective

To examine oral SGLT2 inhibitor use and background factors in elderly heart failure cases in Kyoto Prefecture.

Methods

We analysed 1,483 elderly patients aged 75 years or older (median age 86 years) from the khfnet acute heart failure registry study conducted over one year from July 2022 (1,892 registered cases).

Results

The SGLT2 inhibitor prescription group was younger and had better nutritional status compared to the non-prescription group. Furthermore, examining the association between SGLT2 inhibitor use and prognosis in the ≥75 years and ≥90 years age groups revealed a significantly lower all-cause mortality rate in the prescription group among those aged ≥75 years. A trend towards better prognosis was also observed in the prescription group among those aged ≥90 years. Multivariate analysis indicated that nutritional indicators and left ventricular ejection fraction were associated with SGLT2 inhibitor prescription status. See figure.

Discussion

Appropriate selection of patients for SGLT2 inhibitor prescription may contribute to improved prognosis even in elderly and very elderly patients.
