Facial lipofilling: from historical aspects of its development to modern principles of clinical application

Background. Autologous fat grafting is a minimally invasive technique that combines volumetric restoration with regenerative effects. Despite significant advances in surgical techniques, variability in graft retention and the risk of complications remain important clinical challenges. To summarize and critically analyze the current literature on the anatomical basis, mechanisms of graft survival, technical aspects, emerging technologies, and complications of facial fat grafting. This study was conducted as a narrative literature review. A literature search was performed using the PubMed, Scopus, and Web of Science databases with the following keywords: autologous fat grafting, lipofilling, facial fat grafting, stromal vascular fraction, and nanofat. Reference lists of relevant publications were also screened. The review primarily included systematic reviews, meta-analyses, clinical studies, and landmark publications addressing facial anatomy, mechanisms of fat graft survival, contemporary fat-processing techniques, and complications associated with facial fat grafting. Publications were selected based on their relevance to the review topic, scientific significance, and recency. Interpretation of the published evidence took into account study design, level of evidence, and the availability of systematic reviews and meta-analyses addressing the respective topics. The clinical effectiveness of facial fat grafting depends on the anatomy of facial fat compartments as well as the techniques used for fat harvesting, processing, and implantation. Micro-aliquot injection and atraumatic surgical techniques are associated with improved graft survival. Enrichment of lipoaspirate with the stromal vascular fraction, adipose-derived mesenchymal stem cells, and platelet concentrates may improve clinical outcomes; however, the available evidence remains inconsistent. The use of macrofat, microfat, and nanofat allows tailored restoration of facial volume while improving tissue quality. Although complications are uncommon, severe vascular adverse events may occur. Conclusions. Facial fat grafting is an effective technique in both reconstructive and aesthetic facial surgery. However, clinical outcomes depend on meticulous surgical technique, patient-specific factors, and the selected fat-processing protocol. Despite substantial progress in the field, several aspects of the procedure require further standardization and confirmation in high-quality studies.

Введение. Аутологичная трансплантация жировой ткани является малоинвазивной методикой, сочетающей восстановление объема и регенеративное воздействие. Несмотря на развитие техник, сохраняется вариабельность приживаемости трансплантата и риск осложнений. В статье обобщены и критически проанализированы современные данные литературы, посвященные анатомическим основам, механизмам приживления, техническим аспектам, современным технологиям и осложнениям липофилинга лица. Настоящая работа выполнена в формате обзора литературы. Поиск публикаций проводился в базах данных PubMed, Scopus и Web of Science с использованием ключевых слов: autologous fat grafting, lipofilling, facial fat grafting, stromal vascular fraction, nanofat. Дополнительно анализировались списки литературы релевантных публикаций. При подготовке обзора преимущественно использовались систематические обзоры, мета-анализы, клинические исследования и фундаментальные работы, посвященные анатомическим основам, механизмам приживления, современным технологиям обработки жировой ткани и осложнениям липофилинга лица. Отбор публикаций осуществлялся на основании их соответствия теме обзора, научной значимости и актуальности. При интерпретации опубликованных данных учитывали дизайн исследований, уровень доказательности и наличие систематических обзоров и мета-анализов по рассматриваемым вопросам. Эффективность липофилинга определяется анатомией жировых компартментов, техникой забора, обработки и введения жира. Микродозированное введение и атравматичные техники ассоциированы с более высокой выживаемостью трансплантата. Обогащение липоаспирата СВФ, мезенхимальными стволовыми клетками и тромбоцитарными концентратами потенциально может улучшать клинические результаты, однако данные противоречивы. Использование макро-, микро- и наножира позволяет дифференцированно воздействовать на объем и качество тканей. Осложнения редки, но возможны тяжелые сосудистые события. Заключение. Липофилинг является эффективным методом реконструктивной и эстетической хирургии лица, однако его клинические результаты зависят от соблюдения техники выполнения, индивидуальных особенностей пациента и выбора протокола обработки жировой ткани. Несмотря на значительный прогресс, ряд аспектов метода требует дальнейшей стандартизации и подтверждения в исследованиях высокого методологического качества.
