Objective: To present the experience of endoscopic transnasal reduction of the orbital floor in children and adolescents based on three clinical cases from the Otorhinolaryngology Department of the Russian Children’s Clinical Hospital. Clinical cases: The results of treatment of three patients (aged 5, 7, and 17 years) with isolated orbital floor fractures confirmed by computed tomography were analyzed. All patients underwent endoscopic reduction of bone fragments through the maxillary sinus with temporary balloon tamponade (Foley catheter). Results: In all cases, anatomical restoration of the orbital wall was achieved, and prolapse of peri-orbital fat was eliminated. Control CT showed fragments repositioned to their physiological position. Positive dynamics in the ophthalmological status (resolution of diplopia, restoration of eye mobility) were noted. No complications were recorded. Conclusion: The endoscopic transnasal approach is a safe and effective minimally invasive alternative to open techniques, especially in children and adolescents, as it avoids skin scars and shortens the rehabilitation period. Further studies are required to standardize the protocol.
