A 7-month-old male infant presented with vomiting, upper gastrointestinal bleeding, severe anemia, metabolic acidosis, and shock secondary to a gastric mass. Imaging suggested gastric malignancy with possible splenic involvement, while endoscopic biopsy was nondiagnostic. Complete surgical excision revealed Grade 2 immature gastric teratoma. The patient recovered well following surgery and adjuvant etoposide–cisplatin chemotherapy.
