Evaluation of the efficacy of Unani formulation (Zaroor of Astragalus Sarcacola, Boswellia serrata Roxb., Aristolochia rotunda Linn., Punica granatum Linn.) in comparison to Zaroor-e-Aelwa in shiqaq-e-Miq’ad (anal fissure) patients: A randomised clin

Abstract

Introduction:

An anal fissure, a longitudinal or elliptical tear in the distal anal canal, is the most common cause of severe anal pain. It can be acute or chronic. Currently, surgery is regarded as a gold-standard procedure, but it carries a significant chance of developing anal incontinence, and various pharmaceutical treatments that have been utilized as alternatives also fall short because of their persisting side effects. In the Unani System of Medicine, the term Shiqaq-e-Miq’ad has been coined for fissure in ano, and many Unani formulations are mentioned in classical texts for its conservative management. The objective of the study was to evaluate the effect of Unani formulation ( Zaroor-e-Anzaroot ) in comparison to Zaroor-e-Aelwa in the anal fissure.

Materials and Methods:

This study, entitled “A comparative study of ‘ Zaroor-e-Anzaroot ’ with ‘ Zaroor-e-Aelwa ’ in Shiqaq-e-Miq’d : A randomised clinical trial,” was conducted in the Department of Ilmul Jarahat (Surgery) at NIUM Hospital, Bengaluru, on 60 patients of either sex divided into two equal groups. Blinding was done, and the drug was given for local application with strict dietary restrictions. Subjective and objective parameters were assessed at the baseline and after treatment. Patients were subjected to the topical application of “ Zaroor-e-Anzaroot ,” a compound Unani formulation, advised to apply locally twice daily, and were reviewed every week for the designed period of 3 weeks. The outcomes were analyzed using appropriate statistical methods.

Results:

After 3 weeks of treatment, the present study observed that the most common age group affected was younger and middle-aged adults of the age group 30–40 years, and the incidence in females was slightly greater than in male patients. At the end of the treatment, pain in both the groups completely vanished ( P = 0.001), and complete healing was achieved in all patients.

Conclusion:

Zaroor-e-Anzaroot is as effective as Zaroor-e-Aelwa . Being cost-effective, it can be used as an alternate local treatment for fissures in ano. No adverse effects and associated complications were observed.
