Solitary fibrous tumor (SFT) is a rare mesenchymal neoplasm, accounting for <2% of soft-tissue tumors. Initially described in the pleura, it can arise at various extra pleural sites. Identification of the NAB2–STAT6 fusion gene has improved diagnostic accuracy. Complete surgical excision remains the treatment of choice. A 21-year-old female presented with progressive dysphagia and refractory anemia for six months. Imaging revealed a well-circumscribed intraluminal esophageal mass extending from the cervical to mid-thoracic esophagus and prolapsing into the stomach. The tumor was excised via an open left cervical approach with transgastric extraction. Histology showed spindle cells with CD34 and SMA positivity and nuclear STAT6 positivity, confirming SFT. Recovery was uneventful with no recurrence on follow-up, highlighting the importance of complete excision and immunohistochemical evaluation.
