Ephemeral Eclipse: Transient cortical blindness and complete recovery in posterior reversible encephalopathy syndrome (PRES) – A case report

T Srimathi, M Nivethitha

Posterior reversible encephalopathy syndrome (PRES) is a rare, critical neurological condition characterized by acute neurological and visual disturbances, most frequently associated with hypertension, renal failure, or autoimmune disorders. We present the case of 19-year-old female with history of fever, headache, vomiting, and seizures, followed by sudden, painless bilateral vision loss with elevated blood pressure. Magnetic resonance imaging revealed bilateral parieto-occipital hyperintensities consistent with PRES. Prompt antihypertensive management resulted in rapid and complete visual recovery within 24 hours. The case highlights importance of recognizing PRES as a cause of reversible cortical blindness underscoring the necessity of early diagnosis and intervention for favorable outcomes.
