EP 585 Modified Wise Pattern Therapeutic Reduction Mammoplasty to Adapt Any Quadrant Regardless of Skin Involvement Status

Abstract

Aim

The aim of the study is to evaluate the feasibility, oncological safety, and cosmetic outcomes of using a modified Wise-pattern therapeutic reduction mammoplasty for breast cancer in any quadrant, regardless of skin involvement.

Materials and methods

A retrospective cohort of 250 breast cancer patients with moderate-to-large breasts. All patients underwent modified Wise-pattern therapeutic reduction mammoplasty. Clinicopathological characteristics, surgical details, oncological outcomes, cosmetic evaluation, and patient-reported factors were analyzed. Intraoperative frozen-section assessment was performed for all resection margins to ensure oncological safety.

Result

The median age of the studied patients was 52 years, with a wide distribution across premenopausal and postmenopausal groups. Postoperative outcomes demonstrated that 75% of patients experienced either good or excellent cosmetic results as assessed by both surgeon evaluation and patient-reported satisfaction. The complication rate was low and limited to minor issues such as (0.17 %) delayed wound healing or (0.068%) minor infection, which were successfully managed conservatively. The median follow-up period was 19 months, during which the local recurrence rate was only 0.032%.

Conclusion

Modified Wise-pattern therapeutic reduction mammoplasty is a safe and effective option for breast conservation in medium-to-large breasts, providing excellent oncological control with minimal margin positivity and low recurrence rates. It ensures favourable cosmetic outcomes with low complication rates, making it a reliable technique applicable to all breast quadrants regardless of skin involvement status. This approach allows oncosurgeons to combine oncological safety with superior aesthetic and functional results, supporting its incorporation into routine breast cancer surgical practice.
