EP 407 Sex-Based Differences in Perioperative Outcomes After Laparoscopic Cholecystectomy: A Systematic Review and Meta-Analysis

Aim
Emerging evidence suggests that male gender may be associated with increased intraoperative difficulty of laparoscopic cholecystectomy (LC) and a higher risk of postoperative complications compared to females. This meta-analysis aims to compare outcomes of LC in male patients versus female patients.
Methods
This systematic review was designed, performed, and reported as per the recommendations of the Cochrane Handbook for Systematic Reviews of Interventions and Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Literature databases, including PubMed, Cochrane, Science Direct and Google Scholar, were searched for studies comparing outcomes of LC in male patients versus female patients. The outcomes of interest included operative time, conversion to open surgery, postoperative bile leak, bile duct injury, wound infection, length of hospital stay (LOS), and mortality. The meta-analysis was performed using RevMan version 5.4.
Results
A total of 16 studies were included in this meta-analysis, involving 8,835 patients, with 26.5% being male. The male group showed significantly shorter operative time (MD=8.63, p<0.001), lower rate of conversion to open (OR=2.01, p<0.001), and shorter LOS (MD=0.30, p=0.003) in comparison to the female group. The two groups showed comparable results regarding bile duct injury (OR= 1.96, p=0.180), wound infection (OR=1.02, p=0.940), and mortality (RD=0.00, p=0.950).
Conclusion
Male patients undergoing LC are more likely to experience longer operative times, higher conversion rates, and longer hospital stays compared with females. These results underscore the importance of predicting technical challenges in male patients, while providing reassurance that overall outcomes are safe and beneficial for both genders.
