Enhancing functional longevity: sports medicine facing the challenges of an aging society

Abstract

Modern medicine has successfully extended human life expectancy, yet these additional years are frequently accompanied by chronic disease, functional decline, and loss of independence. This editorial introduces functional longevity as a clinically and socially meaningful framework for healthy aging, defined as the preservation of physical, cognitive, and psychosocial capacity across the lifespan. Moving beyond survival as the primary outcome of healthcare, functional longevity aligns with the World Health Organization’s definition of healthy aging and reflects a rapidly expanding evidence base identifying physical activity as the most powerful modifiable determinant of long-term function. Exercise acts as a systemic biological intervention, influencing metabolic regulation, vascular health, inflammation, and neuroplasticity, thereby reducing the burden of non-communicable diseases, frailty, and cognitive decline. Recent large-scale observational and interventional studies demonstrate that higher levels of physical activity and cardiorespiratory fitness are consistently associated with reduced all-cause mortality, preserved cognitive performance, and delayed onset of disability. Despite this evidence, exercise remains insufficiently integrated into routine medical practice, education, and health policy. The framework of translational exercise biomedicine provides a pathway for converting scientific knowledge into effective clinical and population-level strategies. Within this paradigm, sports medicine is well positioned to lead. Drawing on expertise in exercise physiology, screening, and personalised intervention, sports medicine can bridge the persistent gap between evidence and implementation. Functional longevity should therefore replace survival alone as a key indicator of health-system success. Embedding exercise into clinical care, education, and public policy is not only a scientific necessity but also an ethical and economic imperative for aging societies.
