ABSTRACT

Background:

Overextension of gutta-percha (GP) beyond the apical foramen may impair periapical healing and complicate nonsurgical retreatment. This study compared the efficacy of XP-Endo Finisher R, D-RaCe, and H-files in retrieving overextended GP from the periapical space.

Materials and Methods:

In this in vitro study, sixty-nine extracted human premolars with straight single roots and canals were prepared up to ProTaper F3 and obturated with GP and AH26 sealer using lateral condensation. Overextension was simulated by intentionally extending an F2 GP cone 2 ± 0.5 mm beyond the apical foramen. Specimens were randomly assigned to XP-Endo Finisher R, H-file, and D-RaCe groups ( n = 23 per group). Outcomes were classified as complete removal, attached residual, or displaced/detached material in the simulated periapical space. Descriptive statistics and Pearson’s Chi-square test were used ( P < 0.05).

Results:

Complete removal occurred in 10.1% ( n = 7), attached residual in 40.6% ( n = 28), and displaced/detached material in 49.3% ( n = 34). No significant difference was found for complete removal versus nonremoval ( P = 0.194). However, overall outcome distribution differed significantly among instruments ( χ 2 = 20.842, P < 0.001). D-RaCe demonstrated the highest rate of complete removal, whereas H-files resulted in the highest frequency of apical displacement or detachment of the extruded material.

Conclusion:

None of the tested systems achieved predictable, complete retrieval. D-RaCe showed the most favorable retrieval pattern, whereas H-files tended to displace material apically.
