Efficacy of «Phagodent» gel in radiation-induced oral mucositis

Relevance. Oral mucositis (OM) is a frequent and severe complication of radiation therapy in cancer patients, leading to a reduced quality of life and necessitating interruptions in the treatment of the underlying disease. Existing methods for the prevention and treatment of OM are often insufficiently effective. The aim of this study was to evaluate the efficacy of a comprehensive approach to the prevention and treatment of oral mucositis using Phagodent® bacteriophage gel and individualized radioprotective mouthguards. Material and methods. A prospective randomized controlled trial was conducted at the Russian Scientific Center of Roentgenoradiology from 2023 to 2026, involving 75 patients with squamous cell carcinoma of the oropharyngeal region undergoing radiation or chemoradiation therapy. The patients were divided into three groups (total number of observations n=75): Group I (control; n=25) – patients received basic dental care; Group II (comparison; n=25) – patients received standard therapy for oral mucositis; Group III (main; n=25) – patients received a comprehensive approach using “Phagodent®” gel. Depending on the method of gel application, the main group was further divided into two subgroups: Subgroup IIIa (n=18) – application of “Phagodent®” gel in combination with individual intraoral protective devices (radioprotective mouthguards) to reduce the radiation dose to normal tissues; Subgroup IIIb (n=7) – therapy with “Phagodent®” gel without the use of protective mouthguards. The effectiveness of prevention and treatment was assessed according to the following criteria: clinical presentation (according to the RTOG/EORTC scale), microcirculation parameters, oral microbiota composition, and patient quality of life indicators. Results. In the study group (main; n=25), a statistically significant reduction in the frequency and severity of OM was observed. The comprehensive approach using “Phagodent®” gel and radioprotective mouthguards contributed to the preservation of microcirculation and tissue oxygenation parameters, as well as the targeted reduction of pathogenic microflora. The use of mouthguards provided an additional positive effect. The quality of life of patients in the study group was significantly higher. Conclusion. The developed comprehensive approach utilizing “Phagodent®” gel and individual radioprotective mouthguards is a highly effective method for the prevention and treatment of radiation-induced ОМ, exerting a positive impact on clinical, physiological, and microbiological parameters.

Актуальность. Оральный мукозит (ОМ) – частое и тяжелое осложнение лучевой терапии (ЛТ) у онкологических больных, приводящее к снижению их качества жизни и вынужденным перерывам в лечении основного заболевания. Существующие методы профилактики и терапии ОМ часто недостаточно эффективны. Целью исследования явилась оценка эффективности комплексного подхода к лечению и профилактике ОМ с применением геля на основе бактериофагов «Фагодент®» и индивидуальных радиопротекторных капп. Материал и методы. На базе ФГБУ «Российский научный центр рентгенорадиологии» с 2023–2026 гг. было проведено проспективное рандомизированное контролируемое исследование с участием 75 пациентов с плоскоклеточным раком орофарингеальной области, получавших ЛТ или химиолучевую терапию. Пациенты были разделены на 3 группы: группа I (контрольная; n=25) – пациентам проводили базовый стоматологический уход; группа II (сравнения; n=25) – пациенты получали стандартную терапию ОМ; группа III (основная; n=25) – пациентам применяли комплексный подход с гелем «Фагодент®». В зависимости от способа аппликации геля основная группа была разделена на 2 подгруппы: подгруппа IIIа (n=18) – аппликации геля «Фагодент®» в сочетании с индивидуальными интраоральными защитными устройствами (радиопротек‑ торными каппами) для снижения лучевой нагрузки на интактные ткани; подгруппа IIIб (n=7) – терапия гелем «Фагодент®» без использования защитных капп. Эффективность профилактики и лечения оценивали по следующим критериям: клинической картине (согласно шкале RTOG/EORTC), параметрам микроциркуляции, состоянию микробиоценоза полости рта и показателям качества жизни пациентов. Результаты. В исследуемой группе (основная; n=25) отмечено статистически значимое снижение частоты и тяжести ОМ. Комплексный подход с применением геля «Фагодент®» и радиопротекторными каппами способствовал сохранению параметров микроциркуляции и тканевой оксигенации, а также таргетному снижению патогенной микрофлоры. Применение капп обеспечило дополнительный положительный эффект. Качество жизни пациентов в исследуемой группе было достоверно выше. Заключение. Разработанный комплексный подход с гелем «Фагодент®» и индивидуальными радиопротекторными каппами является высокоэффективным методом профилактики и терапии радиационно-индуцированного ОМ, оказывая положительное воздействие на клинические, физиологические и микробиологические показатели.
