Effects of Non‐Surgical Endodontic Treatment on Inflammatory and Renal Markers in Chronic Kidney Disease and Healthy Patients With Apical Periodontitis

ABSTRACT

This prospective study evaluated changes in high‐sensitivity C‐reactive protein (hs‐CRP) and renal markers like urea, creatinine and estimated glomerular filtration rate (eGFR) following non‐surgical endodontic treatment (NSET) in 37 chronic kidney disease (CKD) and 37 healthy patients with apical periodontitis. Periapical radiographs and serum biomarkers were evaluated at baseline and 6 months using appropriate statistical tests. Following NSET, both groups showed significant reduction in mean PAI score and lesion size. hs‐CRP levels reduced significantly in both groups ( p  < 0.05), with significant proportion of CKD patients in the high‐risk hs‐CRP category declining from 67% to 39% ( p  = 0.006). Blood urea levels decreased significantly in CKD patients ( p  < 0.05). The study demonstrates an association between NSET and improvement in inflammatory and selected renal biomarkers but does not establish causality between apical periodontitis (AP) and CKD. However, timely management of AP in patients with CKD may be clinically prudent and potentially beneficial.
