EFFECTIVENESS OF THE EXTRACELLULAR MATRIX-BASED GEL "SPHEROGEL" IN ORAL SURGERY: RESULTS OF A COMPREHENSIVE CLINICAL AND EXPERIMENTAL STUDY

Optimizing tissue regenerative potential after oral surgery remains one of the most pressing issues in modern dentistry. Despite advances in surgical techniques and technologies, the incidence of postoperative complications remains clinically significant. Objective: To evaluate the clinical and radiographic efficacy of the biomimetic gel based on the extracellular matrix "Spherogel" in various oral surgeries. Methodology. A comprehensive clinical and experimental study was conducted, including an experimental and clinical stage using laser Doppler flowmetry (LDF) and laboratory methods (n = 25) and three parallel randomized controlled clinical trials: removal of impacted third molars (n = 92), root apex resection (n = 42) and dental implantation (n = 36). Results. At the experimental and clinical stage, the use of Spherogel caused a significant improvement in microcirculation parameters with an increase in perfusion by 78.2% (p < 0.001), an increase in the level of osteocalcin by 66.2 % and fibroblast growth factor by 61.9% (p < 0.001). In complex third molar extractions, clinical healing times were reduced nearly twofold, from 19.7 ±4.6 to 10.0 ±1.8 days (p = 4.5×10⁻¹⁵), and complications significantly decreased from 10.9 % to 2.2 %. In root apex resection, radiographic healing was accelerated by 51.6 days (p = 3.0×10⁻⁸). In dental implant placement, osseointegration times were reduced by 33 days (38.8%, p < 0.05), and ISQ scores improved by 26.1 units (51.0%, p < 0.05). Conclusion. Spherogel demonstrates high clinical efficacy in accelerating reparative processes in various types of oral surgery and can be recommended for implementation in clinical dental surgery.
