ABSTRACT

Background and Aims:

Paediatric hip reconstruction produces intense postoperative pain and stress, often requiring opioids. This study evaluated if adding perineural dexamethasone to ropivacaine prolongs pericapsular nerve group (PENG) block duration and modulates postoperative inflammatory stress.

Methods:

In a double-blind randomised trial, 65 children (1–5 years) undergoing hip reconstruction received ultrasound-guided PENG block with 0.2% ropivacaine 0.5 mL/kg plus 0.1 mg/kg dexamethasone (Group A, n = 33) or ropivacaine alone (Group B, n = 32). The primary outcome was time to first rescue analgesia. Secondary outcomes included Face, Legs, Activity, Cry, Consolability (FLACC) scores, opioid consumption, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), blood glucose, and adverse events over 48 hours.

Results:

The median time to first rescue analgesia was not reached in Group A within 48 hours, versus 10.0 hours [95% confidence interval (CI): 8.4 to 11.1] in Group B [hazard ratio (HR) 0.21; P < 0.001]. For children requiring rescue analgesia, the mean time was 15.14 hours in Group A versus 8.90 hours in Group B (mean difference 6.24 hours, 95% CI: 4.87 to 7.61; t (33) =9.24, P < 0.001). Fewer children in Group A required rescue opioids (33.3% versus 75.0%; P = 0.001), with lower 24-hour fentanyl consumption ( P < 0.001). FLACC scores and postoperative rises in NLR and PLR from baseline were lower in Group A (all P < 0.001). Glucose levels and adverse events were comparable.

Conclusion:

Perineural dexamethasone significantly prolongs PENG block analgesia, reduces opioid exposure, and attenuates postoperative increases in NLR and PLR in children undergoing hip reconstruction.
