Aims: This prospective randomized clinical trial aimed to evaluate the effect of a modified cervical surface-treatment protocol to the buccal cervical surface on postoperative cold sensitivity following direct pulp capping. Methods: Sixty patients indicated for direct pulp capping were included and allocated to two groups: control and adhesive. All procedures were performed under rubber dam isolation using an operating microscope by a single experienced operator. After pulp exposure and hemostasis, Endocem MTA was applied, followed by resin-modified glass ionomer cement, and the restorations were completed with composite resin. In the adhesive group, after restoration, the buccal cervical surface was cleaned with airflow, etched, treated with a universal adhesive, and polymerized. Cold sensitivity was assessed baseline, at day 1, week 1, and month 1 using an air-blast test. Patient-reported sensitivity was recorded using the Numerical Rating Scale (NRS), and clinical response was recorded using the Schiff score. Results: No significant differences were found between the groups in baseline NRS or Schiff scores. Both groups showed a significant reduction over time. NRS scores were significantly lower in the adhesive group from day 1 onward, while Schiff scores were significantly lower at 1 week and 1 month. At 1 month, both median scores decreased to 0 in the adhesive group. Conclusion: The modified cervical surface-treatment protocol to the buccal cervical surface may be an effective and clinically feasible approach for reducing postoperative cold sensitivity following direct pulp capping.
