Among the rare morphological variants of ectopic pregnancy, interstitial pregnancy occupies a special place. The diagnostic complexity of this variant is determined by the anatomical and topographic features of the implantation site and the limited information content of ultrasound methods in early gestation. This paper presents a clinical observation of interstitial pregnancy in a 30-year-old female patient with a surgical history of left fallopian tube excision due to pyosalpinx. The patient underwent a comprehensive examination, including a clinical examination, laboratory tests, transvaginal and transabdominal ultrasound scanning, and histological verification of the surgical specimen. Analysis of the presented clinical observation confirmed the ectopic implantation variant—interstitial location of the fertilized egg within the fallopian tube stump extending to the left uterine horn. Laparotomy with excision of the implantation site prevented the development of hemorrhagic complications and preserved the patient’s reproductive potential. This observation highlights the importance of timely surgical intervention as the most effective method for preventing complications and maintaining reproductive health.
