Dynamic Right Ventricular Outflow Tract Obstruction by Membranous Septum Aneurysm: Echocardiography‐Guided Haemodynamic Management. A Case Report

Right ventricular outflow tract obstruction is uncommon in adults but is a recognised cause of haemodynamic instability in congenital heart surgery. We report the case of a 59‐year‐old woman with a 3‐month history of dyspnoea. Transthoracic echocardiogram revealed a subaortic ventricular septal defect associated with a large membranous septal aneurysm protruding into the right ventricular outflow tract, causing dynamic systolic obstruction and moderate‐to‐severe aortic regurgitation. The patient underwent surgical treatment and had a favourable clinical outcome. This case highlights the challenges of maintaining haemodynamic stability and the essential role of transoesophageal echocardiography in the initial assessment, intraoperative management, and postoperative evaluation.
