Distal tubular hyperplasia (DTH) is an underrecognized renal process reported in 5% of patients with end-stage renal disease (ESRD). Herein, we describe a case of 74- year-old man, with ESRD and clear cell papillary renal cell tumor with diffuse tubular distal hyperplasia to highlight this extremely rare finding. Histological examination of the tumor showed features of clear cell papillary renal cell tumor Adjacent renal parenchyma showed multiple DTH’s foci, positive for CK7 and GATA3 with negative AMACR (alpha-methylacyl-CoA racemase) and CD10.
