Mili Doshi, Loraine Macintyre, Danielle Roullier

Good oral health is a vital part of patient care; however, it remains poorly assessed and documented in hospital settings. Nurses and healthcare assistants often lack the training, confidence and structured tools needed to assess and record oral health effectively. This article describes the development and implementation of the Mouth Care Matters Oral Health Assessment and Recording Tool, which has transitioned from a paper form to a digital pathway specifically designed for nursing staff. Created through a multidisciplinary, collaborative process, the tool uses a clear traffic light scoring system that integrates with routine nursing documentation. Developed at Surrey and Sussex Healthcare NHS Trust and piloted across acute NHS settings in England, its use has increased oral health assessments, improved care planning and empowered staff to appropriately escalate concerns. This article highlights the tool's potential to improve oral health outcomes and support the integration of oral care into routine nursing practice.
