Complications and adverse events of cytoreductive surgery in malignant peritoneal lesions

Cytoreductive surgery combined with intraoperative intraperitoneal chemotherapy is the treatment of choice for patients with malignant peritoneal lesions. Objective. To evaluate the validity and safety of surgical interventions in patients with primary and disseminated peritoneal lesions by analyzing the frequency and severity of intra- and postoperative complications. Material and methods. The Abdominal Department of the P.A. Hertzen Moscow Oncology Research Institute has accumulated experience in performing 168 cytoreductive surgeries with various intraoperative antitumor methods in patients with peritoneal carcinomatosis. Among them, 125 surgeries were for pseudomyxoma peritonei, 25 for mesothelioma, and in 18 cases, the indication for intervention was peritoneal metastases of colorectal cancer. Results. Complications and serious adverse events were identified in 31 (18.4%) patients. Grade III—IV complications occurred in 6.5% of patients. Postoperative mortality was 2.9%, which does not exceed the rate for major surgical interventions such as pancreaticoduodenectomy, simultaneous esophageal resection and reconstruction, and extended hemihepatectomy. Conclusion. Cytoreductive surgery for malignant peritoneal lesions is a sufficiently safe and effective treatment method for this category of patients.
