Comparison of the Efficacy of Compression Garments of Different Lengths in Patients with Post-Thrombotic Syndrome of the Lower Extremities: a Prospective, Single-Centre Cohort Study (KODET)

Post-thrombotic syndrome (PTS) of the lower extremities is one of the most severe chronic venous disorders requiring a careful approach to treatment. Compression therapy is the main method of conservative treatment, but the impact of compression garment length on treatment efficacy and patient compliance requires further study. Objective. To evaluate clinical efficacy and adherence to wearing compression garments of different lengths (knee-high or thigh-high stockings) in patients with PTS of the lower extremities. Material and methods. A prospective single-centre cohort study including 98 patients with PTS of the lower extremities (C3—C5 classes according to the CEAP, moderate and severe degree according to the Villalta scale). Patients used class 2 compression garments, knee-high or thigh-high stockings, according to medical recommendations and patients’ preferences. The main evaluation criterion was the change in the total score on the Villalta scale within 6 months. Changes in symptoms’ intensity (pain, heaviness, swelling, and fatigue on a visual analogue scale), treatment adherence and the usability of compression garment were additionally examined. Results. There were no statistically significant differences in clinical efficacy between patients wearing thigh-high or knee-high stockings. Moreover, the use of knee-high stockings was associated with a more significant reduction in heaviness and fatigue (p<0.05) and better garment comfort ratings. Serious adverse events were not reported. Conclusion. Class 2 compression garments significantly decrease symptoms’ intensity and the severity of PTS of the lower limbs regardless of their length (knee-high or thigh-high stockings). Knee-high stockings provide comparable efficacy with a higher level of comfort and convenience that contributes to better therapy compliance.
