Comparative Evaluation of Shade Matching: Smart Monochromatic versus Conventional Multishade Composites using Visual and Photographic Analysis: An In vivo Study

Context:

Creating invisible dental fillings is still one of the most challenging aspects of cosmetic dentistry. Choosing shades for traditional multishade composites is demanding, in contrast to smart monochromatic composites, which claim to simplify the process with structural color matching.

Aims:

To compare the shade-matching ability of the innovative monochromatic composite, Omnichroma, with conventional multishade composites (Filtek Z250XT and Spectrum Universal) using visual and photographic evaluation methods in anterior restorations. “The study groups were divided as follows: Group A consisted of Omnichroma restorations, and Group B included Filtek Z250XT and Spectrum composites, for comparison as multishade composites.”

Materials and Methods:

A clinical study on 30 anterior teeth (two groups, n = 15) restored Class III cavities using Omnichroma, Filtek Z250XT, and Spectrum. Shade match was visually assessed by three evaluators using a Likert scale (0–4). Photographic analysis was done using a Digital Single-Lens Reflex and mobile colorimeter app, converting red, green, and blue to CIE lab values to calculate delta E (ΔE) (color difference). The Chi-square test analyzed visual ratings, while the independent t -test compared mean ΔE values between Omnichroma and multishade composites. Cohen’s d assessed the effect size.

Results:

Visual evaluation by nondentists, dentists with 2 years of experience, and dentists with ≥5 years of experience demonstrated no statistically significant difference between Omnichroma and conventional multishade composites (P > 0.05). The mean ΔE values were 3.6428 for Omnichroma and 4.0795 for the multishade group ( P = 0.471). Although Filtek Z250XT demonstrated the lowest numerical mean ΔE value, the difference was not statistically significant.

Conclusions:

Omnichroma simplified shade matching in anterior restorations and produced clinically satisfactory results. Although Filtek Z250XT showed the lowest numerical mean ΔE value, the difference between groups was not statistically significant. Therefore, Omnichroma and conventional multishade composites showed comparable shade-matching performance within the limitations of this study.
