Comparative evaluation of sciatic obturator femoral nerve block technique versus central neuraxial blockade for anesthesia and post-operative analgesia for below knee orthopedic trauma surgeries

ABSTRACT

Background:

Trauma and Surgery lead to various hemodynamic and metabolic changes occurring in our body affecting the hemostasis is unpredictable manner. We conducted this study considering the blunting of compensatory responses which could be blunted because of the sympathetic block due to spinal anesthesia. In this study we have compared a single puncture technique of major lower limb nerve block [Sciatic, Obturator, Femoral nerve block (S.O.F.T.)] with central neuraxial block.

Method:

It was a prospective randomized study. Group T ( n = 35)- Central neuraxial block; Group S ( n = 35)- S.O.F.T. using single prick technique. The primary objective of the study was to assess and compare sensory and motor characteristic and total analgesic consumption post operatively in 24 hours between the two groups.

Results:

Group S led to significant decrease of mean arterial pressure at 2 min (Group T: 110.2 + 6.9, Group S: 127.9 + 8.1; P < 0.01), 4 min (Group T: 102.5 + 9.7, Group S: 129.5 + 7.7; P < 0.01), and 6 min (Group T: 101.8 + 9.3, Group S: 127.6 + 7.8; P < 0.01). The time of performance was more in Group S (Group T: 2.5 + 0.57, Group S: 12.76 + 0.76; P < 0.005). The onset and duration of S.U.V. sensory block was significantly increased in Group S. The time to first rescue analgesia was greater in Group S.

Conclusion:

The One Prick technique to block Femoral, Sciatic and Obturator Nerve is a feasible and effective technique of anesthesia for successful conduction of below knee surgeries with no hemodynamic fluctuations.
