We report a novel surgical approach combining irido-zonulo- hyaloido-vitrectomy (iZHV) with goniosynechialysis and goniotomy for managing pseudophakic malignant glaucoma (PMG) with extensive peripheral anterior synechiae (PAS). A 51-year-old male presented with uniformly shallow anterior chamber and uncontrolled intraocular pressure (IOP) despite maximum medications. PMG was diagnosed, and iZHV was performed. During surgery, extensive PAS was confirmed, and goniosynechialysis and goniotomy were performed. One year after surgery, IOP was maintained at 12–15 mmHg with a satisfactory anterior chamber depth. Combined iZHV with goniosynechialysis and goniotomy represents a viable and effective surgical option for managing PMG complicated with extensive PAS.
