Coccygeal Intradiscal and Ganglion Impar Radiofrequency Ablation, a Modified Technique for Traumatic Coccydynia: A Case Series

Coccydynia, or tailbone pain, often arising from trauma, leads to sacrococcygeal or intercoccygeal dislocations that may not fully respond to conventional treatments such as ganglion impar radiofrequency ablation (RFA). This article explores a novel approach, combining intradiscal RFA with ganglion impar RFA for the treatment of traumatic coccydynia and hence proposes discogenic pain as a component of traumatic coccydynia. Of the three patients that were selected, two had sacrococcygeal and one had intercoccygeal dislocations. All of them underwent the proposed procedure at 80°C for 90 s. The mean numeric rating score (NRS) pain scores reduced from 8.6 preprocedure to 2.3, 1, and 06, immediate postprocedure, 1-month, and 3-month postprocedure, respectively, which were statistically significant. The improvement in NRS scores was proven statistically significant upon application of paired t -test ( P < 0.05). Analysis of variance test was used to compare the means of all the three postprocedure groups ( P < 0.05). These promising results suggest that this combined technique may offer a more comprehensive treatment option for traumatic coccydynia, though further research with larger sample sizes is needed.
