Clinical efficacy of Ravghan-i-Ikseer in the amelioration of wajaʿ al-Asnān (toothache): A prospective clinical trial

Abstract

Introduction:

Toothache ( Wajaʿ al-Asnān ) is a common cause of orofacial pain that significantly interferes with daily activities such as eating, speaking, and sleeping. Although modern dentistry focuses on eliminating the underlying cause through restorative procedures, immediate access to definitive treatment may not always be available. In Unani medicine, Ravghan-i-Ikseer , a pharmacopeia formulation containing Kafoor Khalis (Cinnamomum camphora), Sat-e-Pudina (Mentha arvensis) , and Sat-e-Ajwāyin (Trachyspermum ammi) , is traditionally used for toothache. Therefore, safe and effective topical agents providing rapid symptomatic relief are of considerable clinical importance. The study aimed to evaluate the safety and clinical efficacy of Ravghan-i-Ikseer in the management of Wajaʿ al-Asnān .

Methods:

A prospective clinical study was conducted at the General Outpatient Department of the Regional Research Institute of Unani Medicine, Chennai. Ninety-eight patients were enrolled, of whom 92 completed the study. Patients aged 18–65 years with a toothache associated with gingival swelling, erythema, bleeding, thermal sensitivity, or headache were included. Five drops of Ravghan-i-Ikseer were locally applied over the affected tooth using a cotton pellet twice daily for 7 days. Clinical assessment was performed on the 3 rd and 7 th days using the Visual Analog Scale (VAS).

Results:

Of the 92 patients analyzed, 85 (92.4%) showed complete relief, 6 (6.5%) partial relief, and 1 (1.1%) no response. A significant reduction in mean overall VAS scores was observed from baseline to day 7.

Conclusion:

Ravghan-i-Ikseer demonstrated significant analgesic and anti-inflammatory effects and may be considered a safe and effective topical Unani formulation for the symptomatic management of Wajaʿ al-Asnān .
