Classical picture of Pigment Dispersion Syndrome with advanced glaucoma

ABSTRACT

Pigment Dispersion Syndrome (PDS) is typically seen in young, myopic males but rarely presents in emmetropic females. We report a 34-year-old woman with classical signs of PDS and advanced glaucoma in the left eye. Examination revealed Krukenberg’s spindle, high intraocular pressure, dense trabecular pigmentation, and near-total optic disc cupping. Gonioscopy confirmed homogenous pigmentation of the angle structures. Despite severe glaucomatous damage, the patient declined surgical intervention. This case underscores the importance of early detection of PDS, even in atypical presentations.
