In patients with a remote history of traumatic brain injury, central adrenal insufficiency may remain unrecognized for decades. An apparently adequate response to standard‐dose ACTH stimulation should not preclude further evaluation of the hypothalamic–pituitary–adrenal axis when clinical and biochemical findings continue to suggest adrenal insufficiency.
