Chondroblastoma of the Hand with an Extensive Soft Tissue Component—A Rare Tumor at a Rare Site—A Case Report

BSTRACT

Chondroblastoma (CBT) is typically located in the epiphysis of long bone of adult. Hand being a rare site of CBT, so commonly confused clinically and radiologically with giant cell tumor of tendon sheath (GCT) and enchondromas which are more common in the hand. We present here a very unusual case of CBT in an elderly male involving third metacarpal and proximal phalanx with an extensive soft tissue component. Initially, the diagnosis of GCT was given on the basis of histopathological examination of biopsy, imaging, and physical examination. However, histopathological examination of the entire specimen revealed features of CBT. CBT-like chondroma, clear cell chondrosarcoma, and CBT-like variant of osteosarcoma are also considered as differential diagnosis because of overlapping clinical, radiological, and histopathological features. Intralesional curettage and bone graft is the treatment of choice of CBT. However in our case, surgeon preferred excision of the entire lesion with the amputation of finger due to extensive nature of swelling to restore the optimal hand movements.
