Carpal Osteomyelitis Caused by Mycobacterium Intracellulare Treated with Vascularized Bone Graft Reconstruction

Hayato Kuno, Shinichi Sasaki, Hiroshi Kuroda

Case:

An 81-year-old man presented with a 5-year history of progressive wrist swelling and diffuse carpal destruction caused by Mycobacterium intracellulare osteomyelitis. Following total carpectomy and temporary cement spacer placement, staged wrist arthrodesis using a vascularized medial femoral condyle bone graft was performed. At 1-year follow-up, solid union was achieved without recurrent infection.

Conclusion:

Nontuberculous mycobacterial osteomyelitis of the wrist is rare and may require staged reconstruction. Vascularized bone grafting may represent a reliable salvage option in destructive infection.
