We describe an exceedingly rare case of an asymptomatic papillary fibroelastoma arising from the right atrial septum in a patient in his early 40s. Furthermore, we retrospectively reviewed previously reported right atrial and interatrial-septal papillary fibroelastoma cases. Papillary fibroelastoma is a benign primary cardiac tumor involving theoretical embolic risk, and right-sided lesions might precipitate pulmonary embolism. Considering the pedunculated morphology and high mobility of the lesion, the patient underwent surgical resection with an uneventful postoperative course free from perioperative embolic complications and remained asymptomatic without thromboembolic events during the 14-month follow-up. Transthoracic echocardiography serves as a robust modality for diagnosing and morphologically characterizing this rare cardiac neoplasm.
