Capturing the invisible: Serial perimetry-based documentation of transient homonymous hemianopia in migraine

Sai Rani Karanam, Sai Arun Karanam, J Sumanth Reddy

A 39-year-old male with a history of migraine without aura presented with new-onset vision disturbance following headache onset, suggesting a possible occipital stroke. Automated perimetry performed during the episode revealed a left homonymous hemianopia, which resolved completely within hours and has been confirmed by repeat perimetry. This case highlights the utility of serial visual field testing in capturing transient visual deficits in migraine with nonclassical temporal relation of aura to headache. Documentation of such reversible visual field changes provides objective evidence of cortical involvement in migraine and aids in differentiating from other acute serious neurological events.
