BSLD-02 PROGNOSTIC ROLE OF VENTRICULAR SIZE IN PATIENTS WITH NEOPLASTIC MENINGITIS FROM SOLID TUMORS AT THE NATIONAL CANCER INSTITUTE

Abstract

Introduction

Neoplastic meningitis is a severe complication of advanced oncologic disease and occurs as a consequence of malignant cell dissemination to the meninges, subarachnoid space, and cerebrospinal fluid. Neuroimaging studies (computed tomography or magnetic resonance imaging) are among the main diagnostic tools. The Evans index is a simple and standardized measurement that reflects ventricular size. Its potential utility in improving prognostic accuracy and optimizing the selection of therapeutic strategies has not been systematically explored.

Primary Endpoint

To determine whether a baseline Evans index greater than 0.30 at the time of neoplastic meningitis (NM) diagnosis is associated with worse overall survival.

Materials and Methods

A retrospective, single-center cohort study was conducted at the National Cancer Institute. The Evans index was digitally measured from axial slices of non-contrast brain computed tomography or magnetic resonance imaging studies. Overall survival was analyzed as the primary outcome using Kaplan–Meier survival curves. Group comparisons were performed using the log-rank test, and a Cox proportional hazards model was used to estimate hazard ratios.

Results

Using a cutoff value of 0.30 for the Evans index, patients with values below this threshold had a median overall survival of 8 months, whereas those with values greater than or equal to 0.30 had a median overall survival of only 4 months (p = 0.003).

Conclusions

An Evans index value greater than or equal to 0.30 measured on neuroimaging at the time of diagnosis is associated with decreased overall survival.
