Bilateral Scleritis and Choroidal Infarction in a Postpartum Patient with Antiphospholipid Syndrome

Purpose:
To report a postpartum case of antiphospholipid syndrome (APS) presenting with bilateral scleritis and choroidal infarction.
Methods:
The patient underwent a comprehensive ophthalmic examination and multimodal imaging.
Results:
A 32-year-old female presented 3 days postpartum with bilateral anterior scleritis, left-sided posterior scleritis, and bilateral choroidal infarcts. Systemic workup revealed elevated cardiolipin and beta-2 glycoprotein I antibodies, leading to a diagnosis of primary APS. Treatment with corticosteroids and anticoagulation resulted in significant visual improvement and anatomical reconstruction of the ellipsoid zone.
Conclusions:
Patients with postpartum APS can present with atypical ocular inflammation. Early recognition is essential for prompt systemic evaluation and treatment to support visual recovery.
