Central retinal vein occlusion (CRVO) is uncommon in young individuals and warrants evaluation for systemic causes. Bilateral ischemic CRVO is particularly rare and suggests an underlying systemic pathology. We report a 34-year-old woman with Eisenmenger syndrome who presented with sudden bilateral severe vision loss and was diagnosed with bilateral ischemic CRVO. Evaluation revealed secondary polycythemia due to chronic hypoxemia. The patient underwent therapeutic phlebotomy and received intravitreal Ozurdex injection in both eyes. This case highlights hyperviscosity as a key mechanism for retinal vascular occlusion in Eisenmenger syndrome and underscores the importance of systemic evaluation in young patients with bilateral CRVO.
