Background:

Anterior lenticonus associated with Alport syndrome presents a unique lenticular pathology in which capsular fragility and distorted anterior lens geometry lead to significant challenges in cataract surgery, biometric measurements, and appropriate intraocular lens selection. This report describes the surgical management of cataract in a patient with Alport syndrome and anterior lenticonus, highlighting the visual and refractive outcomes achieved after extended depth of focus (EDOF) intraocular lens implantation.

Case Presentation:

A 37-year-old male patient clinically diagnosed with Alport syndrome presented with progressive bilateral visual deterioration. Slit-lamp biomicroscopic examination revealed anterior lenticonus associated with cataract formation. Phacoemulsification was performed uneventfully using a carefully planned surgical approach aimed at minimizing capsular stress, followed by in-the-bag implantation of an EDOF intraocular lens. Postoperatively, the patient achieved spectacle independence for distance and intermediate vision with high subjective satisfaction and no dysphotopsia.

Conclusions:

To the best of our knowledge, this case represents the first report describing the use of an EDOF intraocular lens in a patient with Alport syndrome and anterior lenticonus. The findings suggest that EDOF intraocular lenses may be considered as a potential option in carefully selected cases when meticulous surgical planning and technique are employed.
