Beyond the Hype: The Real-world Integration of Artificial Intelligence in Daily Ophthalmic Practice

Abstract

Artificial intelligence (AI) has rapidly transitioned from a topic of academic speculation to a tangible, deeply integrated tool within the modern ophthalmic clinic. As patient volumes increase and the complexity of multimodal imaging grows, ophthalmologists face unprecedented cognitive loads and administrative burdens. This narrative review examines the practical, day-to-day utility of AI across the ophthalmic spectrum. We detail how machine learning algorithms resolve diagnostic ambiguities at the slit lamp, eliminate the “gray zones” in refractive surgery screening, and optimize both preoperative biometry and intraoperative fluidic dynamics in high-volume cataract surgery. Furthermore, we explore the profound time-saving benefits of AI in tracking glaucomatous progression and managing geographic atrophy, as well as the transformative potential of large language models to mitigate physician burnout. By shifting the paradigm from static diagnostics to dynamic clinical and surgical assistance, AI is redefining precision medicine. As these tools enter daily practice, scientific leadership must focus on establishing robust, real-world protocols to ensure equitable and safe implementation.
