“Battling on All Fronts”: A Qualitative Study of Hospital Social Workers in Wartime

Shiri Shinan-Altman, Inbar Levkovich

Abstract

Hospital social workers (HSWs) play a pivotal role during crises by addressing patients’ emotional and social needs under extreme conditions. In this qualitative phenomenological study, we examined the experiences of 25 HSWs during the war in Israel from January to February 2024. Using in-depth semistructured interviews, three key themes were identified via thematic analysis: (1) immediate mobilization to support patients amidst chaos; (2) coping mechanisms ranging from emotional detachment to immersion; and (3) the intersection of personal vulnerability and professional responsibility. Participants described shifting overnight to an emergency routine, serving wherever they were needed, and moving between emotional distance and closeness while worrying about family members serving in the military. Their accounts revealed physical and emotional tolls alongside professional growth and a strengthened sense of mission. These findings highlight HSWs’ resilience and adaptability but also emphasize the need for trauma-informed training, institutional support, and clear protocols to enhance their efficacy in wartime settings. As hospitals move from acute crisis to recovery, these insights can guide how social work departments prepare and support their staff for future emergencies.
