Ayurvedic management of primary infertility with poor endometrial receptivity and leiomyoma: A single case study

ABSTRACT:

Infertility is a growing concern, often linked to poor Endometrial Receptivity (ER) and uterine fibroids. This case study presents the Ayurvedic management of a 30-year-old female with primary infertility who sought treatment at the All India Institute of Ayurveda, Delhi. Despite multiple ovulation induction attempts, she failed to conceive. Clinical assessments, including the Applebaum Uterine Scoring System (13/20), revealed poor ER and a small anterior wall fibroid (13 mm × 12.5 mm). Her hormonal profile and tubal patency were normal. Ayurvedic treatment focused on Vata-Pitta shamana (~alleviating Vata-Pitta dosha ) and enhancing ER through oral medications and Uttara basti (~ basti administered through vaginal or urethral route). A three-day course of Uttara basti using Dadimadi ghrita was administered, along with oral administration of Trikatu churna and Dadimadi ghrita . The patient conceived in the following cycle and delivered a male baby through normal vaginal delivery with no complications. This case highlights the potential role of Ayurvedic interventions in improving ER and addressing fibroid-related infertility through systemic detoxification, uterine tonics, and blood-nourishing therapies. Ayurvedic formulations were instrumental in improving endometrial thickness, blood flow, and implantation rate. The patient reported a positive experience, emphasizing the customized approach of Ayurveda in addressing infertility. This case depicts the need for further research into Ayurvedic management of poor ER as an alternative or adjunct to conventional infertility treatments.
