Association Between Lower Blepharoplasty Surgical Techniques and the Development of Chemosis

Aims. To evaluate the impact of surgical extent in transcutaneous subciliary lower blepharoplasty on the incidence, severity, and course of postoperative chemosis. Material and methods. A retrospective single-center study included 84 patients who underwent transcutaneous subciliary lower blepharoplasty for aesthetic indications between 2020 and 2025. Patients were divided into 2 groups according to the presumed invasiveness of the procedure. The first group included 49 patients who underwent less traumatic procedures, including isolated skin excision and fat pad removal without intervention on the lower eyelid musculo-ligamentous apparatus. The second group included 35 patients who underwent more traumatic procedures involving the lower eyelid musculo-ligamentous apparatus, including myopexy, canthopexy, and skin-muscle flap formation. The severity of chemosis was assessed according to the A.B. Weinfeld classification. Results. Chemosis was observed in 15 (17.9%) of 84 patients. In the group of less traumatic procedures, chemosis developed in 5 (10.2%) patients, whereas in the group with interventions on the lower eyelid musculo-ligamentous apparatus, chemosis occurred in 10 (28.6%) patients. The highest incidence of chemosis was observed after canthopexy, myopexy, and skin-muscle flap formation. Type I chemosis was observed in patients from both groups, whereas type III chemosis occurred only in patients from the second group. No cases of type II or type IV chemosis were recorded. The differences between the groups were statistically significant (p=0.04). Conclusion. The incidence and severity of postoperative chemosis after lower blepharoplasty depend on the extent of surgical intervention. More traumatic procedures involving the lower eyelid musculo-ligamentous apparatus are associated with a higher incidence and longer duration of chemosis compared with procedures limited to skin excision and fat pad removal.

Материал и методы. Проведено ретроспективное одноцентровое исследование, включившее 84 пациента, перенесших чрескожную подресничную нижнюю блефаропластику по эстетическим показаниям в период с 2020 по 2025 г. Пациенты были разделены на 2 группы в зависимости от предполагаемой травматичности вмешательства. В первую группу вошли 49 пациентов после менее травматичных вмешательств, включавших изолированную кожную резекцию и удаление жировых пакетов без вмешательства на мышечно-связочном аппарате нижнего века. Вторую группу составили 35 пациентов после более травматичных вмешательств на мышечно-связочном аппарате нижнего века, включая миопексию, кантопексию и формирование кожно-мышечного лоскута. Тяжесть хемоза оценивали по классификации A.B. Weinfeld. Результаты. Хемоз был зарегистрирован у 15 (17,9%) из 84 пациентов. В группе менее травматичных вмешательств хемоз наблюдался у 5 (10,2%) пациентов, тогда как после вмешательств на мышечно-связочном аппарате нижнего века – у 10 (28,6%) пациентов. Наиболее высокая частота хемоза отмечалась после кантопексии, миопексии и формировании кожно-мышечного лоскута. Хемоз I типа наблюдался у пациентов обеих групп, хемоз III типа только у пациентов второй группы, при этом случаев хемоза II и IV типов зарегистрировано не было. Различия между группами были статистически значимыми (p=0,04). Заключение. Частота и выраженность послеоперационного хемоза после нижней блефаропластики зависят от объема хирургического вмешательства. Более травматичные вмешательства на мышечно-связочном аппарате нижнего века сопровождаются более высокой частотой и большей длительностью хемоза по сравнению с вмешательствами, ограниченными кожной резекцией и удалением жировых пакетов.
