Associated Urinary Tract Infection with Multiple Sclerosis

Abstract

Background:

Multiple sclerosis (MS) causes multilevel uroneurological dysfunction, which increases the risk of urinary tract infections (UTIs) greatly.

Objectives:

The study aimed to investigate the pattern of antibiotic susceptibility of bacteria isolated from UTIs in individuals with MS in Babylon province.

Materials and Methods:

The study collected 80 urine specimens from patients diagnosed with MS admitted to Babil Neurology Center at Imam Sadiq Teaching Hospital between November 2022 and September 2023. The antibiotics susceptibility test was done using the disc diffusion method, employing 20 types of antibiotics from different classes against all bacterial strains.

Results:

The result of the study presents three types of MS among 80 patients. The most prevalent type was relapsing-remitting (71%), followed by secondary progressive (15%), primary progressive (8%), and clinical isolated syndrome (6%). The percentage of females infected with MS was 62 (77.5%), while percentage of males infected with MS was 18 (22.5%). The results illustrate the distribution of the 80 isolated samples (62 females and 18 males) across four age groups: 16–26, 27–37, 38–48, and 49–59, with prevalence rates of 19 (23.75%), 36 (45%), 19 (23.75%), and 6 (7.5%), respectively. The results of the current study found the prevalence of UTI with MS was 56 (70%) from the total 80 samples. Among female patients, the prevalence of UTI infection was 41 (73.2%), whereas among male patients, it was 15 (26.7%). The most prevalent type of Gram-negative bacteria was Escherichia coli , while the more prevalent type of Gram-positive bacteria was Staphylococcus epidermidis . The results of antibiotic susceptibility of Gram-positive bacteria isolates such as Staphylococcus epidermis , Staphylococcus aureus , Streptococcus pyogenes , and Enterococcus spp. showed higher percentage of resistance against specific antibiotics: Gentamicin, Tetracycline (31.5%), Chloramphenicol (23.5%), Tetracycline (83.3%), and Rifampin (50%), respectively. Similarly, the results of antibiotic sensitivity of Gram-negative bacteria isolates such as E. coli , Enterobacter spp., Klebsiella pneumonia , Pseudomonas spp., and Proteus mirabilis showed higher percentage of resistant against specific antibiotics: Amoxicillin/Clavulanic acid (96.2%), Amoxicillin/Clavulanic acid and Ceftazidime (89.4%), Cefixime (65.2%), Ceftazidime (77.7%), and Cefixime (80%), respectively.

Conclusions:

There is a relationship between four types of MS and UTI.
