Assessment of Fracture Resistance in Endodontically Treated Premolars Following Different Access Cavity Designs: “An In vitro Study”

Root canal-treated teeth tend to tolerate functional forces less effectively than vital teeth. This vulnerability is influenced by several variables, including how much dentin is removed and where it is removed during access preparation.
Aim:
This study compared fracture-resistance values in endodontically treated maxillary premolars prepared using traditional endodontic access (TEC), ninja endodontic access (NEC), or truss endodontic access (TREC) access designs.
Materials and Methods:
The samples consisted of 80 extracted human maxillary premolars divided equally into control, TEC, NEC, and TREC groups. After the planned endodontic and restorative procedures were completed, each specimen was loaded in a universal testing machine until fracture. Results:
Intact control teeth had the highest resistance values. Within the treated groups, the mean value was greatest for TREC, then NEC, and lowest for TEC. The difference between TREC and NEC was not statistically significant (
Conclusion:
Under the conditions of this laboratory investigation, NEC and TREC preparations improved fracture resistance compared with TEC. The two ultraconservative access designs did not differ significantly from one another.
